By participating in the Volunteer Leadership Conference, you fully understand all provisions of these policies, and freely and knowingly assume the risk and waive rights concerning liability as described below:

Terms and Conditions

The participant (“Participant”) agrees to indemnify and hold harmless the Cystic Fibrosis Foundation (“CFF”) from all cost, expense, and liability arising out of participation in this Volunteer Leadership Conference  (“Event”); hereby waives and releases all claims, which may be caused by any act, or failure to act, by CFF, its officers, agents, or employees, arising directly or indirectly from participation in this Event; and hereby assumes liability for any loss arising from such participation. Participant acknowledges that Participant (i) has been fully informed of and understands the inherent hazards and risks associated with participating in events of this type; (ii) is qualified, in good health and in proper physical condition; (iii) has the skill and ability to participate in this Event; and (iv) agrees to wear and use all appropriate safety equipment.  Participants enter an area where photography, audio and video recording may occur. By entering the Event premises or Virtual space or otherwise joining Virtually, the Participant consents to such recording media and its release, publication, exhibition, broadcasting or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on websites and any other use deemed appropriate by CFF, and Participant waives all rights to any claims in connection with any such recording, release, publication, exhibition, broadcasting, reproduction or use.  When contemplating Events In-person or Virtual involving physical activity and determining whether Minors (under 18) should participate, parents and guardians should carefully consider whether the nature of the Event contemplated and the abilities of the Minor present any risks and act accordingly.

 

Attendance Policy

Important Note on Attendance at Foundation Events

To help minimize the risk of COVID-19, we ask that attendees at CF Foundation events follow these steps: 

  • Where mandated, practice physical distancing and maintain at least a safe 6-foot distance from persons outside of their household.
  • Face masks are encouraged and should be worn in accordance with local guidelines. (Note, children under two years of age should not wear masks due to safety concerns and therefore should only attend outdoor CF Foundation events where they are able to distance and stay within their family group.)
    • For events hosted by the CF Foundation, masks will be made available on-site, when possible.
  • Follow basic infection, prevention and control practices by regularly washing hands with soap and water or with an alcohol-based hand gel, and by covering your cough or sneeze with a tissue or your inner elbow.
  • Persons with CF should consult their physician before participation in any in-person event as they may be at an increased risk for severe illness from COVID-19.
  • For indoor events, it is CFF policy to invite only one person with CF to be in-person at the event at a time. For outdoor events, people with CF should maintain a safe 6-foot distance from persons outside of their household at all times.

For Your Safety and the Safety of Others

  • For the health and well-being of the CF community, we strongly encourage that participants be fully vaccinated and have received the CDC recommended booster shot when joining us in-person for the event. 
  •  Participants are strongly encouraged to test within 24 hours prior of arrival at an event or meeting, or as close as possible to the time of departure (if traveling). 
  • Please be advised certain events, localities, and venues may require you to be fully vaccinated and show proof of vaccination. We will communicate these requirements in event communications.
  • We are counting on the fact that you appreciate the potential consequences of exposing members of the CF community to COVID-19 and will not attend an event if you test positive, or have been recently exposed to COVID-19.
  • We cannot guarantee that all people present at the event, including participants, sponsors, volunteers, vendors, or others, have been fully vaccinated.

 

COVID-19 Waiver

I hereby forever release and waive my right to bring suit against CFF and its officers, directors, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to attendance at a CFF event. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease, or property losses, or any other loss, including but not limited to claims of negligence, and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen. COVID-19 has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious. The virus spreads from person-to-person contact and/or by contact with contaminated surfaces and objects and/or airborne particles. COVID-19 can cause serious and potentially life-threatening illness and even death. CFF has put in place preventative measures to reduce the spread of COVID-19 at CFF events; however, it is not possible to prevent the presence of the disease and CFF cannot guarantee that you will not become infected with COVID-19. Further, attending a CFF event could increase your risk of contracting COVID-19. Notification of COVID-19 exposure: I agree, by signing below, to notify CFF in the event that I am diagnosed with COVID-19 within 14 days after attending a CFF event. I also agree that CFF may notify other event attendees that an event attendee was diagnosed with COVID-19 after the event. CFF will make every effort to protect my confidentiality, in compliance with relevant state laws, when notifying other event attendees. However, I understand that CFF may be required to also notify the state department of health about my exposure and department of health officials may contact me for further information. Additionally, I agree to immediately notify event management if I begin to feel symptoms of COVID-19 during the event. Assumption of risk: By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending a CFF event. I understand that the risk of becoming exposed to or infected by COVID19 at a CFF event may result from the actions, omissions, or negligence of myself and others, including, but not limited to, CFF employees, volunteers, and event participants and their families.

 

Event Cancellation

The Cystic Fibrosis Foundation reserves the right to cancel this event for any reason including, without limitation, concerns for the health and safety of the Cystic Fibrosis community, weather events, or logistical issues. In the event of a cancellation, or conversion of the event to a virtual event, CFF shall not be liable for any costs of any kind incurred by participants of the event, including, without limitation, hotel and travel costs.

If you included a guest in your registration, please be advised that they will receive a follow-up email asking them to complete the waiver process separately.

Community Guidelines

Virtual events are designed to be safe and open environments for all participants. Please review and follow the guidelines below when participating in a virtual event:

  • Be kind, open-minded, and respectful of everyone’s opinions and life experiences.
    • Do your part to create a judgment-free zone to make the event a positive and meaningful experience for all.
       
  • Keep all breakout and chat discussions confidential.
    • While posting about the event on social media is encouraged, the content of small-group video breakouts and chats is private. Please do not share pictures, screenshots, or information from the chat or breakout sessions in any way.
       
  • Use your real identity.
    • In order to create an open, honest, and transparent community, you must use your real name or initials and upload a picture of yourself to your profile.
       
  • Do not use malicious or offensive speech.
    • Do not use vulgar speech or other language that may make other participants feel uncomfortable. Event participants have different experiences, personalities, and backgrounds. Please do not make assumptions and be sensitive to others’ perspectives.
       
  • Do not ask for or give medical advice.
    • Everyone’s experience with cystic fibrosis and other medical conditions are unique to them. You may learn about how others manage cystic fibrosis or additional diseases as they share their stories, but do not make any changes to your own treatment plan without first consulting your care team.
    • The discussions in the breakout sessions are not intended to be a substitute for medical advice, diagnosis, or treatment recommendations from a qualified professional.
    • If you require medical attention, please consult a clinician, either at your care center or at another treatment institution.

One or more violations of these guidelines may result in your removal from a session, the event, or the virtual environment.

The information you provide during registration will be stored on secure third-party platforms, as it relates to your registration and attendance of this virtual event. The Cystic Fibrosis Foundation may use this information in the future for the Foundation’s general business functions. The CF Foundation will keep any sensitive information it gathers secure and confidential. By registering, you agree that you understand the CF Foundation will handle your information as described in its Privacy Policy.
